Ismail was born and raised in the Egyptian-occupied Gaza Strip, Al-Shati refugee camp. This was after his parents fled Ashkelon, Israel during the 1948 Arab-Israeli War and became refugees in the Al-Shati refugee camp. He lost more than 14 family members in October 2023 during the ongoing Israeli airstrike in Gaza City.

In 1977, he started off by heading Ahmed Yassin’s office and his relationship with Ahmed Yassin grew even stronger over the years. Ismail was later on appointed to serve as the representative of the Palestinian Authority. Allegedly, he was involved in the attacks against Israeli citizens and hence was targeted by the Israel Defense Forces. During the 2003 Jerusalem suicide bombing, Ismail was slightly injured on his hand. Ismail was elected head of the Hamas list in December 2005 which won the Legislative Council elections the following month. In 2016, he took over from Khaled Mashaal in leading Hamas after he won the elections.
